**Please note that priority in registration will be given to those who live within the boundaries of West Park High School**

Football:

·      Early Registration: February 1st to February 28th - $400

·      Regular Registration: March 1st to May 31st - $425

Sideline Cheer:

·      Early Registration: February 1st to February 28th  - $425

·      Regular Registration: March 1st to April 30th - $400

Competition Cheer:

·      Tryouts and Registration in early November

·      Tryouts and registration in early February

Documents are required for all Athletes, including Mascots

  • Please bring all items together. We are not able to accept partial submissions.


1. Medical Clearance Form

  • Must be dated after April 15, 2026
  • Blank attached to this email and on our website under “Downloads”
  • Must include doctor’s stamp and signature
  • If using Kaiser's form, include all pages


2. School Verification Form

Tip: Request this before June, as school offices may be closed over the summer.


3. SAC Code of Conduct Forms (Separate Form for Parent/Guardian and Athlete)


4. Spring Report Card (2025–2026 School Year)


5. Copy of Birth Certificate


6. Volunteer Commitment Deposit – $500 Check (per athlete)

  • Covers required volunteer hours
  • Only cashed if hours are not completed
  • Separate check required for each child
  • Make payable to: West Park Jr Panthers
  • Include child’s name and team in memo (example: Patty Smith 8U Cheer)
  • Postdate check to June 1, 2026


7. Football Equipment Deposit – $300 Check (Football Only)

  • Required for all football players
  • Covers league-issued equipment (uniforms, helmet, shoulder pads)
  • Separate check required for each player
  • Make payable to: West Park Jr Panthers
  • Include child’s name and team in memo (example: Patrick Smith 8U FB)
  • Postdate check to June 1, 2026
